Have you been meaning to fill out an advanced directive but don’t know where to start? SoHum Community Clinic is here to help! Join us at one of our two workshops Wednesday, October 26th from 10:00am-12:00pm or from 5:00pm-7:00pm.
The workshop will include an overview of advanced care options by Dr. Emily Marshall as well as staff to answer questions and assist with paperwork. Public notaries will also be on hand, if and when you are ready to finalize your plans (make sure to bring your driver’s license).
Planning for your medical future is not always easy, but it is important so that loved ones aren’t left having to make difficult decisions on their own. Advanced care planning is an opportunity for you to make your own decisions about what you would like to happen medically, in the event that you are not able verbalize it.
